Course Overview
Incident investigation involves a systematic process of gathering and analyzing information to determine what happened, why it happened, and how to prevent it from happening again.
Root Cause Analysis is a structured method for identifying the underlying causes of an incident. It involves asking “why” questions to identify the root cause(s) and then developing and implementing corrective actions to prevent future incidents.
The ultimate goal of incident investigation and root cause analysis is to improve safety, reduce costs, and enhance overall performance. Properly conducted investigations and analysis can lead to a more proactive approach to safety management and the prevention of future incidents
